I wish to participate in the Trek for Treasure hiking challenge. I understand that complications may arise during hikes. While these complications are rare, they may include death, heart attack, muscle and bone injury. I acknowledge the risks involved in hiking and assume personal responsibility for my health and safety while participating in this program. United General District 304 makes no representation regarding the condition of the trail. All participants must use their own judgement and determine whether the course is safe for their participation in the event. I further release United General District 304 and all participating sponsors from any health problems or injuries that may occur as a result of my participation in this challenge.

I understand that it is my responsibility to consult with a physician prior to and during my participation in the above-mentioned program. I represent and warrant that I have no medical condition that would prevent me from participating in the program. I voluntarily consent to taking part in Trek for Treasure, and understand that I may withdraw from the challenge at any time.

I hereby grant United General District 304 permission to use my likeness in a photograph, video, or other digital media (hereafter called “media”) in any and all of its publications, including web-based publications, without payment or other consideration. I understand and agree that all media will become the property of United General District 304.

I hereby authorize United General District 304 to edit, alter, copy, exhibit, publish, or distribute these media for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. In addition, I waive any right to royalties or other compensation arising or related to the use of the media.

I hereby hold harmless, release, and discharge United General District 304 of all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
